The following terms will provide you with important information policies. Before getting started, please read and sign this form to acknowledge that you have read and accept the following information.
COVID 19 IMPORTANT INFORMATION
HD Fitness have installed a booking system temporarily to control and monitor the number of customers within the premises, We have always been a clean and hygienic gym. Please use the facilities provided and maintain the high hygiene standards in place.

All members must take full responsibility in self decision to return to HD Fitness, HD Fitness will take no responsibility for infection, disease, influenza, cold, or other human to human transmissive diseases.
If you feel unwell or do not accept our Covid measures please wait until further times.
Information and Policies
Customer must wear comfortable workout attire, including, but not limited to, clean t-shirts, shorts, leggings, trousers or tracksuits. Training shoes must be supportive and functional. Workout gloves are optional. Please do not hesitate to ask for advice on what type of clothing and shoes is appropriate & comfortable for you. Strictly no Jeans or abrasive clothing.
Payment is to be made in advance and may be equal monthly automatic payments over the course of the membership. Should Client opt for monthly payments, he/she will automatically billed on the same day every month during the clients membership. Payment dates for all memberships is 1st of each month. There is a one-off joining fee on your first month.If payments are 48hours late your membership will be immediately terminated and all new joining fees will apply. HD Fitness is not responsible if recurring dates vary due to weekends or bank holidays or if you have insufficient funds.
Payment must be completed for all memberships before any new or renewal contracts are rewarded.
Prices quoted on our website, pricelists and elsewhere are in UK Pounds Sterling and are inclusive of VAT at the standard rate of 20%. Prices may increase due to inflation and may decrease due to offers / discounts.
It is the CLIENT’S RESPONSIBILITY to pay his/her membership, even in the event he/she does not attend the gym, if you wish to hold your membership. Places are limited. When a member your payment date is 1st of each month no matter what date you pay. If you wish to cancel your membership we require written confirmation in order to cancel your membership. Should your payment be completed no refunds will be made.
HD Fitness does not guarantee that the collection dates will always fall on the same date each month. HD Fitness is not responsible for any fees incurred if collection dates vary – it is the member’s responsibility to ensure that the required funds are available in his/her account in advance of the collection date or risk the payment defaulting.
If a membership payment is not met, the membership will be terminated with immediate effect and the member will be unable to access the club from that date.
After we have received your membership application, we will advise you of the date of your next and future monthly payments.
Please allow some deviation/leeway for weekends and bank holidays. HD Fitness reserves the right to change its fees at any time and undertakes to advise members of this via email, giving 14 working days’ notice. This allows you time to terminate your contract if you wish. No On-Hold Membership Facility There is no facility for putting pre-paid fixed term HD Fitness gym memberships on hold if you are on holiday, working away from home or in the case of illness or injury. You must pay for your membership to keep your place in the club. However, if this is going to be a long term factor then the only option is to terminate your contract.
However, if you wish to re-join in the future we cannot guarantee you will get your same slot available. As we will only have limited coaching, group and gym slots available each month. We will add you on to our waiting list until the option becomes available. Every time you re-join an induction fee is applied where applicable.
Please note that if we are unable to collect your monthly subscription for any reason – for example, if there are insufficient funds in your bank account or if you change your bank details without advising us – your membership will expire WITH IMMEDIATE EFFECT, just as if you had cancelled your membership. If this happens and you wish to continue using the gym, you must start from scratch and re-join, with possible join FEE.
Cancellation of membership:
You agree to hold full responsibility should you wish to cancel your membership – no refunds or backdating will be made. Following the correct cancellation procedure, as outlined below, if you wish to leave the club.
If you wish to change or cancel a Personal or group training session we need 24hours notice otherwise the session is lost.
If you wish to cancel a class we need strictly 12 hours notice.
To cancel gym membership we need 14 days written notice.
We cannot accept requests to cancel your Monthly membership by verbal / social / communication
It is the client’s full responsibility to cancel all recurring payments. HD Fitness Ltd take no responsibility for uncancelled payments. Membership roll on – All members continue regardless whether you are active or inactive.
Death or Disability:
Should Customer become unable to use or receive services under this contract due to death or disability, Client, or Client’s estate as the case may be, shall be liable only for that portion of the charges allocable to the time prior to death or the onset of disability. Owners require reasonable evidence of death, e.g. death certificate, or disability, e.g. doctor’s note, to be presented at the time of cancellation. Reasonable evidence includes, but is not limited to, a doctor’s letter about the onset of disability or death certificate.
Option to Renew:
Customer shall have the option to renew this contract upon the expiration of the membership. The Client has the option to upgrade membership at anytime during their active membership. Upon contract renewal/ upgrade, HD Fitness may waive the initial registration fee to which all new contracts are subject. By renewing/upgrading a contract, the Client acknowledges and agrees HD Fitness LTD was to great satisfaction including all staff honoured in a professional and courteous manner
The Customer understands that the information collected by HD Fitness will be used for fitness evaluation purposes and for the design, implementation, progression, and maintenance of an individualized fitness program only. I further understand that all such information is confidential and will not be shared with anyone without my prior written authorization, except in the case of a medical emergency or to the minimum extent necessary to achieve a safe and effective fitness program.
All members must be strictly 16+
HD Fitness offers various access systems to enter the gym. Finger print, code, personal card or key fob. If you lose your key fob then you must pay £3 Fee to replace it. Passing your entrance items to anyone else is immediate termination of membership. No refunds will be made.
It is compulsory that you bring a towel to wipe down machines. If you forget a towel you can buy one from reception or use the cleaning products provided on the gym floor.
Anyone who does not return weights, bars etc back to the designated area will be corrected. Failure to ignore the terms of the gym will result in immediate termination of your gym contract and no refund will be allowed.
Personal hygiene is always required . Failure to carry out standard personal hygiene will result in termination of contract / membership should you not apply.
Any member caught spitting whether on gym floor, shower or any area of the gym will be banned from HD Fitness for life. With immediate effect. No explanations, no alternative. You will be refunded the days left on membership and asked to leave.
Completing the required Medical Questionnaire is essential and if any of your medical history changes in the future you need to inform us immediately. Doctor’s letters shall be asked for if there is ill health or an injury that we may be concerned with.
Coaching sessions/ Group sessions:
If you are more than 15minutes late for your Coaching / group training slot we will be unable to proceed with that session therefore the session will be lost.
If you are sick /working abroad or on holidays you must still pay on the same date each month in order to hold your slot. Any coaching or group sessions missed must be caught up by the end of the following month otherwise they will be lost. ( Slots be unavailable should the trainer be busy )
If you terminate your contract by missing a payment, then we will be unable to guarantee your slot times will still be available on return. This is a monthly membership and if you miss sessions it is up to you to catch them up. Memberships are non refundable. If you are injured / sick / working abroad you must catch up any missed sessions. We do not refund the missed slots or discount your membership the following month. Effective from September 2015. Your payment date is the 1st of each month regardless or not whether you have sessions to catch up on. We cannot hold slots unless payment is made on the 1st of each month. If we take any holidays we shall either deduct the time we are away or we will ensure you receive your sessions from another trainer.
Classes must be pre-booked prior to each class. Failure to cancel three classes within a 3 month period results in a one month ban where you are unable to attend any classes. If this happens on a regular occurrence, then a lengthier ban may be imposed. If you have prepaid for that month then this will also be voided and unable to be refunded.
Customer Assumes all risk of participating in an exercise program and agrees that neither HD Fitness nor any of the Trainers will be liable for any injury, illness, or similar difficulty that the client may suffer arising out of or connected with the Clients participation in exercise or a training program at HD Fitness, Dungannon.
Open Access Gym Use
Members that utilize the gym during opening hours while no staff is present will assume all responsibility of risk. Members will not hold HD Fitness, Owners, Trainers, Staff, or Business Liable for any injury occurred during or aggravated by the workout / accident that they conduct at this facility.
Use of security Cameras
HD FITNESS maintain the right to set up and maintain security cameras in Main Gym, Cardio, lobby , reception and exit zones of the Gym. You accept and understand video footage may be taken within the Gym premises for promotion & marketing purposes. Should you wish footage to be removed whether picture image or video please contact a member of staff.
All clients are responsible for cleaning up after themselves. Sanitary spray and paper towels are provided by the business in order to keep equipment clean and Customers safe.
The Client may be required to have a Medical Clearance and Physicians Consent form signed prior to starting any training. Please complete the medical form below. If you have any

a. Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?
b. Do you feel pain in your chest when you do physical activity?
c. In the past month, have you had chest pain when you are not doing physical activity?
d. Do you lose your balance because of dizziness or do you ever lose consciousness?
e. Do you have a bone or joint problem (for example, back, neck, knee, or hip) that could be made worse by a change in your physical activity?
f. Is your doctor currently prescribing drugs (for example, water pills) for your blood pressure or heart condition?
g. Do you know any other reason why you should not do physical activity?
h. Have you any drug implications for training whether pharmaceutical or recreational that may hinder your health during exercise ?
i. Do you have an abnormal resting Heart Rate ?
j. Are you Diabetic?
k. Do you have Hyperlipidaemia (cholesterol >220mg/dl or total Cholesterol-to-HDL ration of >5.0?
